 PUBLIC RECORDS REQUEST

Requests for public records can be made in person at the Little Chute Safety Center, by telephone (920) 788-7505, by fax (920) 788-7385, by US mail. You may also make a records request by email to:

To facilitate your request, complete the Request for Public Records form available below or at the department’s lobby service window.

DEPARTMENT PUBLIC RECORDS REQUEST FORM

The department will charge a fee for the actual, necessary, and direct costs of reproduction of records. Payment is due upon the receipt of the records
 
 * NOTE: Fees in excess of $5.00 will require prepayment.

 Permissible charges include:

  • Copying and reproduction
  • Location, if exceeds $50.00
  • Mail and shipping
  • Computer programming needed to locate and produce records
  • Other specified in 19.35(3) Wis. Statutes

FEE SCHEDULE 
 
The department shall respond to records requests as soon as practicable, as outlined in §19.35(4) (a), Wis. Stats.  Absent exceptional circumstances, a request should be fulfilled within 10 business days.  Should a request take longer to fulfill, the requestor shall be notified of the reason for the delay and provided with an estimated date of completion.  The actual, necessary, and direct costs of reproduction of records shall be assessed.

To obtain Driver's License records or Vehicle Registration records, a Vehicle/Driver Record Information Request form must be completed and mailed to:

To obtain criminal history records, a Criminal History Record request form must be completed and mailed to:
